To be eligible to be added to the Wait List for enrollment to the NA/HHA program, documentation of clinical requirements for immunizations and CPR certification must be on file at the Health Sciences office. Documentation for these Clinical Requirements must be submitted as a complete packet in a 9" x 11" envelope.
Records will not be accepted separately.
Print the Part 1 - Pre-Enrollment Clinical Requirements checklist. Use this checklist as your guide in completing all of the necessary steps. Proceed with Steps A - D. When complete, bring your materials (in the 9" x 11" envelope) to the the Health Sciences Division Office.
Once you have completed Part 1 Clinical Requirements and have the required documentation for each item on your checklist, the packet may be submitted to the Health Sciences office. The Division secretary will review the materials and verify completion. Please bring your original records along with a copy for the Health Science Division records. Your original records will be returned to you upon verification.
Your name will be added to the Wait List based on the date you submit the completed clinical requirement materials for Steps A-D.
Step A - Immunizations - document on the Immunizations /TB screening Documentation.
Step B - CPR - As a nurse aide student, you will be assigned to patients in healthcare facilities for your clinical experiences and as such are required to be currently certified (certification must be valid throughout the course in which you are enrolling) in Basic Life Support with CPR for the Healthcare Professional which includes CPR training for adults, children, and infants. Training and testing will be with appropriate manikins and will include one and two person rescue. To document current CPR certification, bring your original CPR certification card along with a copy that you will submit in your packet.
Step C - Technical Standards - Nursing is a physically demanding profession and you must be prepared to meet the technical standards necessary to physically perform the functions of nursing. Upon review of the standards, print the Technical Standards Disclosure Form, initialing each of the areas in which compliance with the standard is possible.
Step D - Confidential Clinical Application - The initial step to completing the criminal background check is to print and complete the Health Science Program Application for Clinical Practice form. This form discloses to Shasta College (Confidentially) the existence of any criminal issues. While the existence of convictions will not necessarily disqualify you from enrollment, failure to disclose may be considered falisfication and will result in the offer of enrollment being rescinded and will be grounds for immediate termination upon discovery at any time during enrollment.
