2017 WELLNESS INCENTIVE:
For 2017 STSIG will use just one form to report all activities. please use the 2017 Wellness Tracker Form.pdf
. They will not be using Anthem or Navitus reports for wellness activities so every activity must be reported on this form. This will help with timely reporting of activities. Their goal is to have the wellness activities recorded in PlanSource within 1-2 weeks of receiving the forms.
Once you have completed the attached form, please return the completed form to Leah Grant by email at firstname.lastname@example.org or by fax to (530) 221-6225. The completed form must be submitted by October 31, 2016
in order to receive the 2017 wellness incentive.

New: Anthem will be sending each subscriber with dependents an annual dependent questionnaire in the subscriber’s birthday month. The questionnaire will ask about if any of the dependents have other medical coverage. If the questionnaire is not returned, Anthem will not process dependent claims until the questionnaire is completed and submitted to them. It is extremely important that you complete and return the letter even if your family members do not have other coverage.
PRESTIGE - Special Member Alert Announcement!
Attention employees who receive medical benefits through Shasta College! We are very excited to announce the STSIG’s partnership with Prestige Urgent Care beginning April 4, 2016. The details can be found on the flyer: PRESTIGE - Special Member Alert Announcement - March 2016.
A special note: You must make an appointment with Prestige unless it is urgent care. For urgent care it is requested that members call ahead of time. Please remember to tell the front desk you are a member of Shasta-Treinity Schools Insurance Group at check-in.
New BENEFIT added: Prestige Urgent Care has offered to allow spouses and dependents of current members not enrolled in our medical plans an opportunity to join at a discounted membership cost. This is another great added benefit to our members. Enrollees will need to refer to this offer specifically when enrolling to receive the discount. Grand Rounds Announcement!
Grand Rounds is a benefits offered free of charge to our members enrolled in our medical program. Grand Rounds can be used to receive second opinions and schedule office visits with in-network specialists. For more information, please visit www.grandrounds.com/sisc or call 1-844-252-3056.
Open Enrollment is from October 26th through November 20th. Employees will use PlanSource for their health benefits elections. This is an ACTIVE open enrollment which means that each employee must choose a new plan for 2016 from the new plan options available. All PlanSource login passwords will default to the birthday format on October 12th. Passwords expire every 60 days. Plan options and information will be emailed to active employees on October 9, 2015. Please see below for the entire 2016 Active Employee Open Enrollment packet.
4th Quarter Carry Over:
The 80%/20% plans feature Fourth Quarter Carryover on the Deductible beginning October 1, 2016. Covered charges incurred from October through December and applied toward the Calendar Year Deductible for that year also counts toward the Calendar Year Deductible for the next year. Fourth Quarter Carryover does not apply to Calendar Year Out of Pocket Maximum.
Dental and Vision:
- The district continues to cover the full cost of dental and vision premiums for the 2016 short plan year.
- Dental and Vision, unlike medical plans, are still on a composite rate for the 2016 short plan year.
- Dependents can now remain on the plan until age 26 (was previously 25).

. The deadline for completing the wellness incentive requirements in October 31, 2015. Members who have their wellness exams in September and October always seem to not make on the Anthem reports in time for the incentive to show up on PlanSource before the October 31st deadline. This year we encourage everyone having their wellness exam to submit proof of the exam directly to STSIG/JPA office (this is optional). Members can submit a receipt from the doctor’s office or use the Bio-metrics form which now has a check box for the doctor to mark for the wellness exam. This will help to avoid confusion.

savings account that completed the wellness incentive for 2016 will
receive a $100 gift card on January 31, 2016.
- High deductible plan members with a

This year’s health fair will be on Tuesday, October 13, 2015 from 5pm to 8pm at Sequoia Middle School, McLaughlin Auditorium, 1805 Sequoia Street, Redding. Registration is required. Click here to register. Attending this event will earn wellness incentive for 2016.

2015 Medical Plan Information:
All benefit eligible employees will need to schedule a one on one appointment to meet with a Benefit Counselor by visiting www.myenrollmentschedule.com/shasta
or by calling (866) 998-2915. Benefit counselors will go over your CORE enrollment (medical/dental/vision), your Section 125 pretax medical premiums, your Section 125 Flexible Spending and Dependent Day Care, as well as your Voluntary Benefit options such as life insurance, critical illness insurance, accident and disability insurance.
