American Fidelity Section 125 and Voluntary Benefits One on One Appointments

. American Fidelity will review available Section 125 plan options being offered to you using the new online enrollment system allowing you to authorize your Section 125 pretax benefit election. You will also be educated on voluntary Section 125 Flexible Spending Account (FSA) options available, and Day Care Account to cover other “out of pocket” medical, dental and vision expenses or day care expenses that may help reduce your gross taxable income. During the meeting, you will have the opportunity to receive more information from American Fidelity on voluntary benefit options that may be important to you and your family such as disability insurance, life insurance, Critical Illness, Cancer and Accident Insurance. There will no longer be new enrollments into Trustmark or Unum plans. Please note that Shasta College will be discontinuing payroll deduction for Trustmark plans, Unum Short and Long-Disability plans, and Wageworks products with the last regular payroll of the year December 18, 2015. If you wish to continue with your Trustmark or Unum policies you will need to make arrangements with them directly. If you do not make arrangements with them directly, your policies will automatically expire on December 31, 2015.
American Fidelity Section 125 and Voluntary Benefits Group Meetings
American Fidelity will be on campus to give an overview of Section 125 Benefit program, optional voluntary benefits and answer questions at the Shasta Campus Theatre on Tuesday, October 20th at 3:00pm and Wednesday, October 21stat 8:30am. This is a great opportunity to get a high level overview on voluntary Section 125 Flexible Spending (FSA) options available, and Day Care Account to cover other “out of pocket” medical, dental and vision expenses or day care expenses that may help reduce your gross taxable income. During the general meeting, you will also get an overview of other voluntary benefit options that may be important to you and your family such as disability insurance, life insurance, Critical Illness, Cancer and Accident Insurance. You can then come prepared to your one on one appointment knowing which Section 125 and voluntary plans that you would like to learn more about and/or which plans you would like to sign up for.

Open Enrollment is from October 26th through November 20th. Employees will use PlanSource for their health benefits elections. This is an ACTIVE open enrollment which means that each employee must choose a new plan for 2016 from the new plan options available. All PlanSource login passwords will default to the birthday format on October 12th. Passwords expire every 60 days. Plan options and information will be emailed to active employees on October 9, 2015. Please see below for the entire 2016 Active Employee Open Enrollment packet.

It’s not too late to earn the wellness incentive for 2016. For more information, please visit the STSIG 2016 Wellness Incentive page at http://www.stsigjpa.com/html/Wellness_2016.htm
. The deadline for completing the wellness incentive requirements in October 31, 2015. Members who have their wellness exams in September and October always seem to not make on the Anthem reports in time for the incentive to show up on PlanSource before the October 31st deadline. This year we encourage everyone having their wellness exam to submit proof of the exam directly to STSIG/JPA office (this is optional). Members can submit a receipt from the doctor’s office or use the Bio-metrics form which now has a check box for the doctor to mark for the wellness exam. This will help to avoid confusion.
This year’s health fair will be on Tuesday, October 13, 2015 from 5pm to 8pm at Sequoia Middle School, McLaughlin Auditorium, 1805 Sequoia Street, Redding. Registration is required. Click here to register. Attending this event will earn wellness incentive for 2016.

2015 Medical Plan Information:
All benefit eligible employees will need to schedule a one on one appointment to meet with a Benefit Counselor by visiting www.myenrollmentschedule.com/shasta
or by calling (866) 998-2915. Benefit counselors will go over your CORE enrollment (medical/dental/vision), your Section 125 pretax medical premiums, your Section 125 Flexible Spending and Dependent Day Care, as well as your Voluntary Benefit options such as life insurance, critical illness insurance, accident and disability insurance.
