January 1 - September 30, 2016 Medical Plan Information:

New:  Anthem will be sending each subscriber with dependents an annual dependent questionnaire in the subscriber’s birthday month.   The questionnaire will ask about if any of the dependents have other medical coverage.  If the questionnaire is not returned, Anthem will not process dependent claims until the questionnaire is completed and submitted to them.    It is extremely important that you complete and return the letter even if your family members do not have other coverage.

Early Retiree Wellness Incentive

If you are an early retiree not on Medicare, it's not too soon to start earning the wellness incentive for 2016.  For more information, please visit the STSIG 2016 Wellness incentive page at http://www.stsigjpa.com/html/Wellness_2016.htm.  The deadline for completing the wellness incentive requirements is October 31, 2015.

You will be mailed this information on October 8th.
