SLO Forms

The SLO reporting forms are now in an interactive online format. These online forms allow you to turn in SLO data without having to open/save any documents. The questions are the same as on the old forms.

 

1) For Spring 2010 Courses:  Course SLO Assessment Reporting Form. 

All instructors need to complete this form for each course taught for every semester. These forms are due no later than one week after grades are due. To preview the questions before you fill out the electronic form,  click here for a sample form .


 
2) For Fall 2009 Courses Only: Course SLO Assessment Reporting Form. 

All instructors need to complete this form for each course taught for every semester. These forms are due no later than one week after grades are due.

3) Summary SLO Assessment Reporting Form.  

This form should be used after Flex Day to report faculty observations and plans for improvement based on SLO assessments for the academic year (e.g. report 2009-2010 summaries in Fall 2010). If you are the only instructor teaching a particular course, you still must submit this summary report. This form is due no later than the 3rd Monday in September.

4) General Education SLO Candidate Assessment Form 

Use this form to document GE course cross-discliplinary SLO methods proposed by faculty on Flex Day, Spring 2010.

5) Degree/Certificate SLO Assessment Form 

Use this form to document Degree & Certificate SLOs and the assessment method to be used for each.
