The SLO reporting forms are now in an interactive online format. These online forms allow you to turn in SLO data without having to open/save any documents. The questions are the same as on the old forms.

This form should be used after Flex
Day to report faculty observations and plans for improvement based on
SLO assessments for the academic year (e.g. report 2009-2010
summaries in Fall 2010). If you are the only instructor teaching a
particular course, you still must submit this summary report. This form is due no later than the 3rd Monday in September.
Use this form to document GE course cross-discliplinary SLO methods proposed by faculty on Flex Day, Spring 2010.
