A couple of pointers for completing the form

  • When you open the assessment submission form, you will be asked for your MyShasta/Outlook username and password.
  • Complete the form for each section you taught in Fall 2021. You can return to the form using the link above, or select the option after submitting results to fill out the form again.
  • Keep your course syllabi open so you can cut and paste the relevant SLOs into the form.

How to save copies of your completed forms

  1. Before submitting your completed form, check the box at the bottom of the second page that says “Send me an e-mail receipt of my responses.” 
  2. You will receive an e-mail from Microsoft Forms. Open it and click on “View My Responses.”
  3. This will take you back to your completed form.
  4. Right click and select “Print.”
  5. Open the drop-down menu of printer options, and select “Save as PDF.”
  6. Save your completed form to your computer (you may want to change the file name to indicate your course name and section number).

What if I’m part-time?

Part-time faculty members are eligible to receive an additional 2 hours of FLEX pay per semester for completing SLO work. If you have any questions about the assessment process in your department or area, please contact your area coordinator.
